Canon ef rf focusing problem

    r6 - ef 70-200 2.8 ii - sigma 24 1.4
    I have a Canon R6 and I'm using the ef rf adapter with the ef 70-200 ii.
    I've been using the different focus modes and have found that when I use anything but small center square it gives me bad results.
    The weird thing is with the eye tracking, when I'm looking at the pictures on the back of the camera, the red focus box is telling me its on the eye when its hitting about a ft behind that every time.
    Anyone experiencing this?
    I also used the Sigma 24 1.4 and I can't see any problems with this lens which makes me think it's my 70-200 :(
    After my shoot today I just updated the firmware to 1.5 but I don't think I saw anything relevant in the firmare notes.
